After you translate your Video Title and Description, in Step 4 you will be able to modify your translated titles and descriptions.
What you need to do is simply edit the text, include different links (for example) in descriptions, make tweaks...
Once done, click on SAVE from the bottom of the screen.
Once the modifications are saved, you can go ahead and hit Publish to publish the translations on YouTube.
